Glenoid Ligament Tear with Capsular Detachment

Definition:
An injury to the glenoid labrum (fibrocartilaginous rim attached around the margin of the glenoid cavity) where the labrum detaches from the glenoid fossa.
Symptoms:
  • Pain, usually with overhead activities
  • Catching, locking, popping, or grinding
  • Occasional night pain or pain with daily activities
  • A sense of instability in the shoulder
  • Decreased range of motion
  • Loss of strength
Causes:
  • Falling on an outstretched arm
  • A direct blow to the shoulder
  • A sudden pull, such as when trying to lift a heavy object
  • A violent overhead reach, such as when trying to stop a fall or slide
Treatment:
  • Period of immobilization in a sling
  • Passive ROM to the shoulder, then progress to active ROM
  • Posture program
  • Shoulder stabilization exercises
  • Upper extremity strengthening
